Award Categories

Individual Award

About the award:

The "Individual" Award is for someone in your nursing team who performs beyond their general job description.  This award aims to recognise the significant contribution in raising standards of care to our patients and service users in Primary Care and have made an outstanding contribution.

Who can be nominated:

Entries are accepted for individuals working in your nursing team and who are actively practicing in LLR (at the time of nomination). 
Entries will be accepted from Nursing staff nominating themselves or their team member. Non-nursing colleagues such as managers or GPs may also nominate their nursing colleagues for an award.

Criteria:

The nominee should clearly demonstrate:

• commitment to person-centred care, delivering high quality services that make a difference to the people receiving care in Primary Care
• evidence of innovation within their practice and community, improving positive outcome for patients, families and colleagues
• how they excel at co-worker support, how they actively seek ways to support nurses and other colleagues helping others to grow 
• how their work contributes to the delivery of local and national policy and strategy within Primary Care

Team Award

About the award:

This award is for an outstanding team which really makes a difference to our patients.  The award recognises Nursing teams or a multidisciplinary team where the nursing role has been instrumental in the success of a project or service delivery. 

• Do all members function as a truly cohesive team while celebrating and representing diversity?
• Do they have complementary skills and abilities?
• Are they open, communicative and helpful?
• Do they consistently supply an excellent service to patients/carers/staff?

Who can be nominated?

Entries are accepted from nurses nominating their own teams or can be submitted on behalf of another team. Non-nursing colleagues such as managers or GPs may also nominate their nursing colleagues for this award. 

Criteria:

The nomination should clearly demonstrate:

• the team’s commitment to person-centred care, delivering high-quality services that make a difference to the people in Primary Care
• the team that has impacted on positive outcomes in the practice setting and/or community through innovation and enterprising healthcare initiatives 
• evidence of working in a cohesive team whilst celebrating and representing diversity
• how their work contributes to the delivery of local and national policy and strategy within Primary Care
